A scoping review of 27 studies across sub-Saharan Africa found that approximately 68% of countries rely on Gavi donations — often supplemented by the Gates Foundation, WHO, or other donors — as their primary HPV vaccine financing mechanism, with financial delivery costs ranging from US$0.27 to US$19.76 per dose depending on country, delivery modality, and vaccine type. Single-dose schedules consistently demonstrated lower financial and economic costs while maintaining comparable efficacy, and school-based delivery proved cost-effective in multiple contexts. Key financing barriers include heavy dependence on external donors, limited locally generated evidence for financial planning, high delivery costs, and difficulty demonstrating HPV cost-effectiveness relative to competing health priorities. The review concludes that African governments must integrate HPV vaccination into national budgets, adopt cost-effective procurement practices, and tailor delivery strategies to local contexts to achieve the WHO 90-70-90 elimination targets sustainably.
